Contact details, broad inquiry type, preferred contact method and other high-level qualifiers may be enough for the first step.
Keep the first form focused
Every extra field adds friction and data-handling responsibility. Ask why the firm needs each field now rather than later.
Separate public intake from detailed case information
More sensitive or detailed information can be collected later through an appropriate process once the firm determines what is necessary.
Connect form data to action
A field is valuable when it changes routing, qualification, scheduling or follow-up. Otherwise it may simply create database clutter.
Test mobile completion
Many prospects will complete forms on phones. Keep labels clear, inputs touch-friendly and the form short enough to finish without frustration.
